What do we do?

We conduct Annual Reviews of Education, Health and Care (EHC) Plans to ensure that each child or young person continues to receive the support they need to achieve their outcomes. As students approach Year 9 and beyond, our reviews place a strong emphasis on Preparation for Adulthood, covering:

  • Employment and further education

  • Independent living

  • Community participation

  • Health and wellbeing

We work collaboratively with families, professionals and agencies across the education, health and care sectors, and the young person to ensure the EHC Plan remains relevant, ambitious, and in line with their evolving aspirations.

How do we do it?

Our Annual Review process is person-centred, inclusive, and forward-looking. We:

  • Arrange the review meeting six to eight weeks in advance, to provide everyone plenty of time to prepare for it and make arrangements to attend.

  • Invite any relevant professional and/or agency.

  • Gather views from the young person, their family, and professionals ahead of the review meeting.

  • Share what information we received in advance of the meeting, so parents have an opportunity to prepare themselves properly.

  • Hold a structured review meeting that celebrates progress, identifies challenges, and updates outcomes.  Invite the student to attend part of the meeting where appropriate.

  • Focus on Preparation for Adulthood by exploring interests, strengths, and future goals in areas such as college, work experience, independent or supported living, and social life and community participation.  The focus on Preparation for Adulthood starts at Y7 but intensifies at Y9 and every year thereafter.

  • For students in their final year at Walton Hall Academy, we discuss and plan the transition process and support that will be provided by school.

  • Make recommendations for updates to the EHC Plan to reflect any changes in needs, provision, aspirations and/or outcomes.

  • Coordinate with external agencies such as careers advisors, social care, and health services to ensure a joined-up approach.

  • Submit the report to the local authority ensuring everyone who was invited to the meeting receives a copy.

  • Seek feedback from parents and carers about the review process and make any improvements necessary as part of an evaluation process.

Why do we do it?

Annual Reviews are a legal requirement, but more importantly, they are a vital opportunity to:

  • Empower young people to shape their own future.

  • Ensure continued relevance of the EHC Plan as needs and goals evolve.

  • Promote independence and inclusion, helping young people prepare for life beyond school.

  • Strengthen partnerships between education, health, social care, and families, by ensuring joint person-centred working.

  • Secure and support transition into further education, training, employment, or adult services.

By embedding Preparation for Adulthood into every review we help young people build the confidence, skills, and support networks they need to flourish in life after school.
